I hereby certify that the statement and personal information data I have given above are true and correct of my own knowledge; that I agree that, should any of such statements our data to be false, the Manila Resource Development Corporation shall have the right to reject (outright) this application and forfeit all lawful fees that I might have paid and that if I have already been actually employed, the said Corporation or its foreign Principal of their agents shall have the right to cancel, terminate, or rescind my employment contract and that I forfeit any benefits due me under said contract.

I further certify that I have not given any amount or other valuable consideration to the said Corporation, or any of its officers/employers for this application, and that I understand that I am to be issued that corresponding Official Receipt duly signed by an Officer of the said Corporation for any and all amount will be paid by me.

I also declare and agree that I shall not be entitled to any refund of any amount, if refund is proper, unless the amount I claim is supported by any Official Receipt of the Corporation, and I finally declare that I fully understand and that the said Corporation has not promised or guaranteed my employment abroad by virtue of this application.
